Semaglutide is an FDA-approved prescription drug (Ozempic and Rybelsus for diabetes, Wegovy injection and Wegovy pill for weight management, with additional approvals for cardiovascular risk reduction, kidney disease in type 2 diabetes and MASH with liver fibrosis) and is fully legal to prescribe and dispense in the United States. It is not a controlled substance and is not prohibited by WADA.
Compounded semaglutide is a different matter. Compounding of a commercially available drug is generally allowed only under specific conditions, most notably during an official FDA drug shortage. The FDA declared the semaglutide shortage resolved on February 21, 2025. Its grace period for state-licensed pharmacies ended April 22, 2025, and for outsourcing facilities May 22, 2025. In April 2026 the FDA spelled out when a compounded version counts as an unlawful copy of the approved drug, and on August 27, 2026 the Fifth Circuit upheld the FDA's shortage decision. Compounding is now limited to narrow, patient-specific cases, and compounded semaglutide is not FDA-approved. Anyone considering a compounded version should confirm current status with a licensed pharmacy and clinician.
